I will be voluntary participating in the fitness services that will be conducted by the trainers. I am fully aware that engaging in physical exercise and the use of exercise equipment, and participation of any class could cause injury ranging from minor scrapes, strains, and bruises to significant injuries such as broken bones, eye injury or loss, concussions or even death. I hereby release Volkz Jiu Jitsu Academy’s, owners, and its employees from liability for injury, death, or property loss suffered by me resulting, while using the facilities, equipment, or in any way associated with participating in all activities now and in the future.
